If you want to use your mental health benefits for office visits, please note that it is your responsibility to verify and understand your benefits/coverage. I am an in-network mental health insurance provider for Blue Cross Blue Shield PPO policies, the N.C. State Health Plan, and the Federal Employee Plan. 

CPT insurance codes that I typically use for billing purposes are as follows: 90791 (initial visit for all patients); 90837 (therapy sessions); and 96136, 96137, 96130, and 96131(testing).

Achievement testing is not covered by insurance policies and is paid out of pocket. I recommend that you verify your mental health benefits for the services in which you are interested by calling your insurance company prior to calling me.          

Please also determine if any prior approval or pre-authorization is needed from your insurance company before your first office visit. If prior approval is needed, all paperwork needs to be completed with a response from your insurance company before your first office visit; otherwise, you will be responsible for full payment of services provided.          

Additionally, please note that I am not an in-network mental health insurance provider for Blue Local or Blue Cross Blue Shield HMO policies. If your medical benefits are with Blue Cross, but they contract out your mental health benefits to another company (e.g., United Behavioral Health, Value Options, Cigna), I would not be an in-network provider for you.          

New patients should review the following office documents:

1)  Business Policy

2)  North Carolina Notice Form 

If we schedule an appointment, review and complete the two following documents and bring to the first office visit: 

1)  Receipt of Notice / Permission for Evaluation and/or Treatment 

2)  Insurance Authorization Form  

* If you are interested in a psychoeducational evaluation with me, please bring copies of previous testing reports, standardized testing results, and report cards to the first visit as well. 

** If you want me to release any information from you or your minor child’s office records to a third party, please complete the Release of Information form and mail or fax to me at the office.
